Coaching Agreement Form
Coaching is a service that promotes and delivers expanded self-awareness and personal responsibility and empowerment leading to motivated and focused self-directed positive actions and behaviours resulting in desired progressive changes in peoples’ lives.
As such, though I will facilitate all of the above with my coaching skills, tools and techniques, all and every choice and decision made by you as a result of your coaching sessions, is solely your responsibility. I will not at any time seek to impose my views overtly or otherwise and will not instruct nor direct you to take any actions. I may offer suggestions or options from time to time that will be in line with your options on the understanding that you will only follow through with them if you can take ownership of them.
You may ask about, and I may suggest an amount of sessions, that might be required to allow you to achieve your goals or outcomes, but you will always have the right and responsibility to determine the ending of the coaching relationship, notwithstanding my own rights to end the coaching sessions should I determine that it is no longer professional or safe to continue. As you would expect, our coaching sessions are completely confidential, although of course subject to our constitutional laws which require knowledge of any unlawful activity to be reported to the relevant authorities and includes the danger of harm to self or others.
All written information relating to your sessions, including your contact details, are covered by the guidelines issued by the Data Protection Agency and are compliant with the new GDPR regulations (enforced by the EU from May 25th 2018).

Accreditation and Your Contact Details
Consent
To become an accredited member of the International Coach Federation (ICF), I would need to hold onto your email address as I may be subject to a random audit by them at some point in the future should I gain my accreditation. If this occurs, the ICF would have access to your email address (which I need to hold onto for this purpose) and could email you to check the accuracy of my coaching log details (amount of sessions, dates, payment). To give your consent to this, please include current email address and sign below.
